All students who participate in co-curricular activities will be responsible for reading and following the
Co-Curricular Code of Conduct. Additionally, students and parents will be responsible for signing the
Activities Eligibility Form prior to attending or participating in any co-curricular events. Every student participating in activities must sign this code annually.
Transportation Waiver Form
